Once you have activated all the SIM cards you received, please let us know the Optus phone number or numbers you chose.
This information helps us to check you're getting the free recharges. We won’t give the phone number or numbers to anyone outside of The Smith Family, or Optus. They are only for the purpose of providing the recharges. We also won't automatically update these details on our records. If you want to update your mobile number, you can do this on My Smith Family.
For each person in your family who received a new Optus Donate Your Data™ SIM card, put their phone number in the box below their name.
When you have entered all the phone numbers, click submit.
